Detailed Terms and Conditions governing the provision of nutritional therapy services are issued to clients upon engagement and form part of the contractual agreement.
To provide mutual protection and reference throughout the client-practitioner relationship, clients must sign the Terms of Engagement agreement and provide it to the practitioner along with their full payment at the time of booking. The practitioner also signs this agreement and shares the mutually signed document with the client. Consultations are unable to take place without this agreement being signed by both parties.

Nutritional Therapy Descriptor
Nutritional Therapy is the application of nutrition science in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners use a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address nutritional balance and help support the body towards maintaining health.
Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Registered Nutritional Therapy Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a ‘one size fits all’ approach. Registered Nutritional Therapy Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with ‘red flag’ signs or symptoms to their medical professional. They will also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client’s care to explain any nutritional therapy programme that has been provided.
Your registered Nutritional Therapy Practitioner at Create Health Clinic takes a collaborative approach. Client and practitioner will discuss and agree on client goals, level of support, and course of action in the first consultation and review this at subsequent sessions throughout the programme.
Your Practitioner
Sarah Thomas is a fully qualified, UK-regulated Nutritional Therapy Practitioner, registered with the British Association of Applied Nutrition & Nutritional Therapy (BANT) and the Complementary and Natural Healthcare Council (CNHC). Sarah Thomas was awarded her Level 6 Diploma in Nutritional Therapy with distinction from the British College of Nutrition and Health (BCNH).
Payments
All fees for programmes, consultations, and clinic credit are payable in full and in advance. Payment must be received before any intake forms are completed, personal health data is shared, or appointment times are reserved. An invoice will be issued at the time of booking, and payment confirms and secures the booking.
Payments can be made by credit or debit card using the secure payment link provided on your invoice. If you wish to pay by bank transfer, this must be discussed with your practitioner in advance.
Programme Cancellations
Programme cancellations must be made in writing at least 72 hours (3 days) before the first scheduled consultation.
Cancellations made within this timeframe are eligible for a refund of the total amount paid, less a £150 administration fee.
Cancellations made with less than 72 hours’ notice before the first scheduled consultation, or after a programme has partially commenced are not eligible for any refund.
This does not affect your statutory rights.
